Collaborating to deliver better care

The idea of establishing a clinically integrated network in North Platte was fostered through the leadership and vision of our local physicians. Physician leaders saw the need for a formalized structure that supports the delivery of high-quality, low-cost patient care through teamwork, transparency and the use of data analytics. The Great Plains Health Innovation Network (GPHIN) was the result of this vision, and the collaborative efforts that followed its inception in 2016 have demonstrated compelling results in improving care for the patients we are privileged to serve.

The goal of GPHIN is to foster innovation and provide the necessary support to improve the overall coordination of care, prevention of disease and management of chronic illness. Our value-based care payer partners work in collaboration with the Great Plains Health leaders to provide GPHIN with the necessary infrastructure and financial support to leverage technology and strengthen our utilization management processes.

Care coordination services

GPHIN officially introduced care coordination to our network members in 2018 with services that help our providers by bringing focused education and resources to patients for preventive care, chronic disease management and health coaching. Stories of patients who have benefited from this additional care are truly heartwarming and vary from as simple as providing a low-income diabetic patient with assistance managing meals by accessing healthy foods from local food pantries to transporting very complex patients to long-distance appointments to ensure that they will qualify for the surgery necessary to improve their life. We often see our Care Coordinators acting in place of family members for patients who are without support, and we are thrilled about the positive impact they are making on patient lives while simultaneously reducing the costs of care.

GPHIN practice recognized as Centers of Distinction for Total Care

In May of 2018, Blue Cross Blue Shield of Nebraska recognized GPHIN practices as Blue Distinction Total Care providers. Blue Distinction® Total Care recognizes doctors who focus on healthcare instead of sick care. These doctors go above and beyond to enhance the overall health of their patients, providing preventive services and wellness coaching, as well as working with patients with chronic conditions to better meet their care needs. Total Care is coordinated, patient-focused and, in many cases, more affordable healthcare.

My Life, My Choice

Great Plains Health advance care planning

GPHIN is launching a new program to help our communities embrace advance care planning to ensure that all patients have the opportunity to decide how they would like their care handled near end of life. The program is titled My Life, My Choice and will provide education and resources to optimize end-of-life experiences for both patients and their family members. The plan includes training staff on best practices for educating our community on the benefits of Advance Care Planning (ACP).

Our Post-Acute Advisory Committee members are partnering with GPHIN on this journey to provide education and assistance through staff training, patient identification and family counseling.
