Your Medical Record

Your medical record is a compilation of pertinent facts of your health history, including past and present illnesses and treatments, written by the health care professionals contributing to your care.

It includes all inpatient, outpatient, emergency and clinic records. As a patient at Great Plains Health, you are assigned a medical record unit number (MRUN) on your first visit to our facility. This number is retained for all subsequent services and all of your records are maintained in a unit record in the Health Information Management Department.

Ownership of your medical record

Your medical record is the property of Great Plains Health and shall be maintained to serve you, the patient. Physicians and other health care providers who are involved in your care and treatment utilize it.

As a patient, the information contained in your medical record belongs to you. You are entitled to the protected right of informationm, and Great Plains Health maintains your records in accordance with legal, accrediting and regulatory agency requirements.

Copies of your records may be made available to you, but requests cannot be received or sent via email due to HIPAA privacy and security concerns.

Your privacy is protected and you will be given the opportunity at registration to be listed in the facility directory; send notices of your admission/discharge to the local newspaper; or participate in the Patient Identification Number (PIN) Program, which allows those you choose to receive detailed medical information regarding your stay. You may also choose not to participate in any of the programs and keep your information private.

Great Plains Health also participates in the Nebraska Health Information Initiative, in which information is sent to a protected statewide patient information database that is available to your physician or other health care provider. Those patients who do not wish to participate should contact the registration department when scheduled or receiving treatment.
